- Download Form EPIC-002 from the Chief Electoral Officers website. This is the application form for applying for Duplicate Voter ID.
- Fill the necessary details like your name, relatives name, residential address, area etc. in the application.
- Visit your nearest election office to submit duly signed application and copy of FIR related to complaint of loss of Voter ID, Aadhaar card etc.
- Reference ID number will be given after submission of application. This is necessary to know the status of the application. After receiving the application you will get a message from the election office to receive the duplicate voter ID.
How to Apply Offline for Duplicate Voter ID?
- Visit the nearest election office
- Ask for Form EPIC-002
- Enter your name, original voter ID number, gender and age, home address, area of residence, police station etc. Also, tick the right side for options related to return of damaged voter ID card and return of duplicate ID after recovery of lost voter ID card.
